Interviu/Interview – Prof. Dr. Torello Lotti

Avem deosebita plăcere să discutăm cu Prof. Dr. Torello Lotti, președintele Academiei Mondiale a Sănătății – Secțiunea Dermatologie, prorectorul Universității „Guglielmo Marconi” din Italia și unul dintre cei mai prestigioși medici din domeniul dermatologiei și venerologiei.

We have the special pleasure of talking with Prof. Dr. Torello Lotti, president of the World Health Academy – Dermatology Section, the vice-chancellor of the University ”Guglielmo Marconi” from Italia and one of the most prestigious doctors in the field of dermatology and venereology.

Maria Diana Focșa: Sunteți președintele Academiei Mondiale a Sănătății – Secțiunea Dermatologie, profesor la numeroase universități prestigioase din întreaga lume. Cum a început totul, ce v-a determinat să alegeți medicina și mai cu seamă să deveniți medic dermatolog?

You are the president of the World Health Academy – Dermatology Section, professor at many prestigious universities around the world. How did it all start, what led you to choose medicine and especially to become a dermatologist?

Prof. Torello Lotti: Când m-am înscris la Universitatea din Florența, după liceu, m-am gândit și la Facultatea de Științe Politice, sperând să am șansa de a deschide vreodată ușa unei cariere diplomatice. În schimb, în ultimul moment, m-am hotărât pentru Medicină. Același lucru a fost valabil și pentru specializare. M-au impresionat studiile privind culoarea pielii, care au fost realizate în cadrul Departamentului de Dermatologie. Cred că așa m-am hotărât să devin dermatolog.

When I enrolled at the University of Florence after high school, I also thought about the Faculty of Political Science, hoping to have the chance to open the door to a diplomatic career someday. Instead, at the last minute, I decided on Medicine. The same was true for specialization. I was impressed by the skin color studies that were done in the Department of Dermatology. I think that’s how I decided to become a dermatologist.

M.D.F.: Ne puteți numi câteva persoane cu o influență semnificativă în viața dumneavoastră, care v-au fost modele și ce ați învățat de la dânsele?

Can you name a few people with a significant influence in your life, who were your role models and what did you learn from them?

Prof.T.L.: Mentorul și maestrul meu a fost profesorul Emiliano Panconesi, fondatorul, primul președinte și președintele primului congres al Academiei Europene de Dermatologie și Venerologie, o personalitate cu totul deosebită. Nu a fost doar un om de știință, ci a fost și scriitor și pictor. Datorită lui, am devenit redactor-șef al „Revistei Academiei Europene de Dermatologie și Venereologie” și am găsit suficientă energie pentru a deveni profesor titular de dermatologie când eram încă destul de tânăr. Trebuie să-i mulțumesc profesorului Emiliano Panconesi pentru că m-a inspirat.

My mentor and master was Professor Emiliano Panconesi, founder, first president and president of the first congress of the European Academy of Dermatology and Venereology, a very special personality. He was not only a man of science, but he was also a writer and a painter. Thanks to him, I became the chief editor of the ”Journal of the European Academy of Dermatology and Venereology”and found enough energy to become a full professor of dermatology when I was still quite young. I must thank Professor Emiliano Panconesi for inspiring me.

M.D.F.:Care au fost cele mai frumoase momente din cariera dumneavoastră?

What were the best moments in your career?

Prof.T.L.: Sincer, nu știu care au fost cele mai bune momente din cariera mea. Ceea ce m-a impresionat cu adevărat au fost sentimentele care au apărut și s-au dezvoltat împreună cu rezidenții mei, șansa de a transfera cunoștințele către generația mai tânără și ideea de a crea o comunitate în comunitate.

Honestly, I don’t know what the best moments of my career have been. What really impressed me were the feelings that arose and developed with my residents, the chance to transfer knowledge to the younger generation and the idea of creating a community within the community.

M.D.F.: Revista TIMPUL Medical se adresează în principal publicului larg. Care sunt cele mai des întâlnite afecțiuni din domeniul dermatologiei și ce sfaturi le dați cititorilor noștri pentru a preveni apariția acestor boli?

TIMPUL Medical magazine is mainly addressed to the general public. What are the most common conditions in the field of dermatology and what advice do you give our readers to prevent the occurrence of these diseases?

Prof.T.L.: Da, sunt multe boli de piele care devin tot mai comune. În primul rând, aș vrea să vorbesc despre cancerul de piele, în special cel non-melanom. Pentru aceasta, cea mai bună metodă este prevenirea, iar educația pentru expunerea la razele solare este esențială. Dat fiind acest fapt, nu trebuie să uităm că nici măcar cele mai puternice loțiuni nu ne pot garanta faptul că nu vom face cancer de piele. Așadar, pentru cele din imaginile 1-3, expunerea la razele solare trebuie să fie foarte redusă. Un alt aspect important este melanomul , caz în care expunerea la soare este mai controversată, cu toate acestea, este necesară și în acest caz monitorizarea de către un dermatolog. Alte boli de piele tot mai comune sunt cele inflamatorii cronice autoimune precum psoriazisul, vitiligo și dermatita atopică. Există multe noutăți în domeniu, iar în acest moment putem oferi pacienților soluții atât pentru prezent cât și pentru viitor. Nu în ultimul rând, mai există și așa numitele boli estetice, care, în realitate, au prea puțin de-a face cu aspectul fizic. Dintre acestea amintim formele de alopecie și acneea. Tot aici se încadrează și vitiligo. Chiar și în cazul acestor afecțiuni, au apărut noi oportunități de tratament și diagnoză. În cazul vitiligo, FDA a anunțat în premieră un nou tratament aplicat local, și anume Ruxolitnib 1,5%.

Yes, there are many skin diseases which are becoming more and more common. First of all, I would like to start with skin cancer, mainly non-melanoma skin cancer. For this the best way is prevention and education to exposure to the sun rays is essential. In any case, we should never forget that creams which protect us even at the highest levels of protection are never enough for making us free of getting skin cancers. So for photo types 1 to 3, limitation to sun exposure should be very very low. Another important issue is melanoma, in which case sun exposure is more controversial, but still we should always keep it under control of an expert dermatologist.  Other conditions that are becoming more and more common are autoimmune inflamatory chronic skin conditions like psoriasis, vitiligo and atopic dermatitis. There are many big news for this and, at this moment, we can provide excellent tools for our patients, both for the present and for the future. Lastly, the so- called aestethic disorders, which are not aesthetic at all, like acne or hair loss, mainly alopecia, or vitiligo. Even for these disorders, there are new chances for diagnosis and treatment. For vitiligo in particular, the FDA  has recently made a statement for clearing existing the first local treatment which is only for vitiligo – Ruxolitinib 1,5%.

M.D.F.: În aprilie se desfășoară o nouă ediție a Primăverii Dermatologice Ieșene (PDI), iar Revista TIMPUL Medical are bucuria de a fi parteneră media a acestui eveniment. Cum vi se pare acest eveniment medical cu tradiție?

In April, a new edition of the Iași Dermatological Spring (PDI) takes place, and TIMPUL Medical Magazine is happy to be the media partner of this event. What do you think of this traditional medical event?

Prof.T.L.: PDI este un eveniment educațional extraordinar care are loc sub auspiciul doamnei profesor Daciana Brănișteanu. Sunt bucuros  să fac parte din comitetul de experți. Congresul PDI diferă de alte congrese prin ADN-ul său. De fapt, vorbim de un congres cu adevărat interdisciplinar, care oferă tuturor șansa de a înțelege mai bine bolile de piele și legătura dintre acestea și corpul uman, precum și cu alte boli. Cu alte cuvinte, nu trebuie să uităm că ființa umană funcționează ca o armonie. Atunci când studiem pielea, nu putem face abstracție de organe și viceversa. Iar acest fapt este adesea ignorat în majoritatea întâlnirilor medicale. Această premisă reprezintă nucleul congresului PDI. Îmi doresc ca acest congres să dăinuie mulți ani de acum încolo și să înflorească.

PDI is an extraordinary  educational event which takes place under the direction of professor Daciana Brănișteanu. I’m very happy to be part of the faculty and the expert committee. What makes this PDI extraordinary is its DNA, which is totally different from other congresses. In fact, this is a truly interdisciplinary congress, which gives  everyone the chance to gain better knowledge about the disorders of the skin and their relationships with the entire body and all the other disorders. In other terms, we should never forget that the human being is a unity. The skin can never be  disconnected by other organs and viceversa. This is what it is forgotten in too many medical meetings, this is the central core of the PDI congress and for this I can say long life for the PDI congress. May the PDI grow and flourish.

M.D.F.: PDI se desfășoară în Iași, cum vi s-a părut orașul Iași când l-ați vizitat prima oară?

PDI takes place in Iași, how did you like the city of Iași when you visited it for the first time?

Prof.T.L.: Orașul Iași este minunat. Ceea ce impresionează cu adevărat este centrul vechi, unde te poți bucura de savoarea istorică, educațională și culturală a mai multor secole, precum și vechea universitate. Oamenii sunt foarte prietenoși, iar orașul reușește să îmbine armonios arhitectura stilată a secolelor trecute cu abordarea arhitecturală modernă. Asta face ca Iașul să fie perla României.

The city of Iasi is extraordinary. What really makes it impressive is the downtown, the old part of the city, where you can find the flavor of many centuries of history, education, culture and the long lasting university. The people are very friendly and the city is able to put together the most beautiful old stylish buildings with the modern part which makes it like a diamond of the beautiful Romania.

M.D.F. : La sfârșitul interviului, puteți oferi câteva sfaturi tinerilor medici dermatologi?

At the end of the interview, can you give some advice to young dermatologists?

Prof.T.L.: Cred că le pot oferi adevăratul mecanism de înțelegere a dermatologiei. Pielea este centrul acestei vederi holistice a omului, singura vedere care poate aduce din ce în ce mai departe medicina și chirurgia.

I think I can give them the real mechanism of understanding dermatology. The skin is the center of this holistic view of man, the only view that can bring medicine and surgery ever further.
